Booster Shot Clinic-Large GYM

­Our partners at Greater Cleveland Pediatrics have agreed to host another vaccine clinic for the Pfizer Covid Bivalent Booster and Flu Vaccine next week. Note: This is the same clinic Jim referenced in a prior email – the clinic date had to be moved up due to changes in the doctors’ schedules. All eligible community members and immediate family 12 years of age or older are invited to pa­­rticipate in this clinic. If you wish to register yourself and/or an immediate family member for the clinic, please follow the registration directions below.

Part 1: Insurance Information and Consent

Please complete demographic information and insurance information in full. It is very important that all of this information is entered in prior to your vaccination time so that we can efficiently vaccinate. Please complete the medical screening questionnaire.
 
Part 2: Appointment and Vaccine Selection
 
Information from GCP Regarding Pfizer Covid19 Bivalent Booster: Can be given to individuals who are 12+ years old and have received an initial Covid19 vaccine series and any number of previous boosters, as long as it has been at least 2 months since your last Covid19 Vaccine. It is okay to mix and match between Pfizer/Moderna brands for this booster. If you have had Covid19 in the past 3 months, you may still receive a Bivalent booster vaccine or you may consider waiting until 3 months following your recent infection. We recommend that you review your eligibility with your personal doctor if you have questions. This vaccine cannot be given to individuals with a history of severe anaphylaxis to a prior Covid19 vaccine. There is no charge for the Pfizer Covid19 Bivalent Booster, though insurance information will be collected if available.

Information from GCP Regarding Flu Vaccine: The seasonal flu vaccine can be given to individuals 12-65 years old. A different vaccine is recommended for individuals ages 65+. You should not receive this vaccine if you have a history of Guillain-Barre Syndrome. You may receive the Covid19 Bivalent Booster and the Flu Vaccine on the same day. If “In-Network” with Vaxcare, there will be no charge. Insurance information will be collected for the Flu Vaccine. If “Out-of-Network” with Vaxcare, you may pay a $25 cash fee to receive the flu vaccine on-site at AOA.
