Application Procedure: Preschool to Grade 4

1. Application for Admission: Complete and return the application with the $40 application fee and a copy of the applicant’s birth certificate (required by the State).
2. Candidate Questionnaire: The applicant must complete this form without assistance (Grades 3-4).
3. Parent/Guardian Questionnaire: Comments and insights from parent(s)/guardian(s) are welcome and necessary.
4. Academic Recommendation: We request that you give the academic recommendation to an educator of choice. In addition to the form, please provide the teacher with a postage-paid envelope.
5. Transcript Release: Parent or guardian must sign this form and submit it to the applicant’s current school. The school will release the information directly to Andrews Osborne Academy.
6. Admission Testing: Please contact the Office of Admission to schedule an assessment.
7. Campus Tour: It is important for applicants and parents or guardians to visit the campus. Please call the Office of Admission to arrange a visit.
8. Financial Assistance: If your family plans to apply for financial assistance, please indicate so by marking the appropriate place on your application. We will send you a Parent Financial Statement (PFS). Financial Assistance is only available to U.S. citizens in grades 1-12.

Included in this application packet are the forms you will need to apply for Grades 5-12 at Andrews Osborne Academy.
1. Application for Admission: Complete and return application to the Office of Admission along with the $40 application fee. The fee for international students is $50.
2. Student Questionnaire: You must complete this form without assistance.
3. Parent/Guardian Questionnaire: Comments and insights from your parent(s)/guardian(s) are welcome and necessary.
4. Confidential Recommendations: We request that you give the confidential recommendation forms to the appropriate teachers. You should provide your teachers with stamped envelopes addressed to the Andrews Osborne Academy Office of Admission.
5. Transcript Release: Parent or guardian must sign this form and submit it to the applicant's current school. The applicant's school will release the information directly to Andrews Osborne Academy.
6. Admission Testing: Please register to take one of the following tests in order to complete the application process. You may take either the Independent School Entrance Exam (ISEE) or the Secondary School Admission Test (SSAT). International students whose first language is not English must submit the results of the Test of English as a Foreign Language (TOEFL).
Secondary School Admissions Test (SSAT) School Code: 1274
The SSAT is administered by the Secondary School Admissions Testing Board. They can be reached at www.ssat.org or 609-683-4440.
Independent School Entrance Exam (ISEE) School Code: 365565
The ISEE is administered by the Educational Records Bureau. They can be reached at www.erbtest.org or 800-989-3721 ext. 312.
Andrews Osborne Academy will offer the Independent Entrance Exam (ISEE) on the following dates during the 2011-2012 academic year:
Saturday, December 10, 2011 (register by November 19, 2011)
Saturday, January 21, 2012 (register by December 20, 2012)
Saturday, February 18, 2012 (register by January 28, 2012)
Saturday, April 14, 2012 (register by March 24, 2012)

7. Interview and Campus Tour: It is important for you and your parents or guardians to visit Andrews Osborne Academy for an interview and tour of the campus. Please call the Office of Admission as soon as possible to arrange this visit. All applicants for grades 5-12 to Andrews Osborne Academy must complete an Admission Interview prior to being admitted to the school.
